- If paying by check, print completed registration form and mail with check to JBS & Associates at 954 La Mirada Street, Laguna Beach CA 92651. Please make all checks payable to: JBS & Associates. Receipts will be e-mailed to the address listed on this form. One registrant per check, or list multiple registrant names on the check stub.
Payment on behalf of a group: If you are paying on behalf of a group, each participant should complete a registration form so conference staff can create name tags and a participant list.

Cancellations
Conference registration cancellations received in writing by March 23, 2012 will be refunded less a $50.00 service fee.
Transfers/substitutions: There is a $25.00 service fee to transfer a paid registration to another person. Deadline for transfers is March 23, 2012.
