conference registrationYou may choose to register using either a fill-in PDF (then fax or mail) or use the online registration system this year. In prior years some attendees have been concerned about faxing or mailing credit card information so the conference is migrating to using an online secure web-based registration. The registration link below will take you to the 2010 conference secure, encrypted registration web site where you can register using a credit card. However, if you prefer, you may still use the traditional fill-in PDF registration method. Fill in this form and fax or mail or e-mail it to Patty at JBS & Associates (address, phone number and fax number are on the form). Only register once.
payment methodFor payment of registration fees, the conference will accept payment by corporate check, personal check, VISA or MasterCard. Your credit card billing statement will read "HMO Conference." We prefer to receive a separate check for each registration. However, if your organization must write one check for group registrants, kindly note on the check the names of the individuals paid by the check. Send in your registration forms before the deadline date, even if the payment follows later. Make checks payable to JBS & Associates. Registration forms can be phoned in, mailed, or faxed to:
